Final Days to Secure Feb.  1 Health Insurance Coverage with Peak Health Alliance and Elevate Health Plans

Nonprofit Partnership Urges Coloradans to Act Before Jan.  15 Deadline

Peak Health Alliance and Elevate Health Plans by Denver Health Medical Plan, two Colorado-based nonprofit organizations, are urging Coloradans in nine rural counties to act now to secure health insurance for 2025.

Open enrollment for coverage starting Feb. 1 ends Wednesday, Jan. 15, and those who miss this deadline risk being uninsured for the year.

By working together, these nonprofits ensure accessible, budget-friendly health insurance options tailored to the needs of Coloradans, including:

  • Lowest premiums for individual plans in Dolores, Grand, Lake, and San Juan counties.
  • The lowest-cost High Deductible Health Plan options in Archuleta, Park, La Plata, Montezuma, and Summit counties.

How to Enroll

Coloradans can explore their options and sign up for coverage through Connect for Health Colorado or directly via Elevate Health Plans by Denver Health Medical Plan’s website. For personalized help, call 970-455-0382 to speak with a knowledgeable support team member who can guide you through the enrollment process.

“Time is running out to secure health coverage starting Feb. 1,” said Rich Cimino, executive director of Peak Health Alliance. “As community-focused nonprofits, we’re committed to ensuring every rural Coloradan has access to affordable and comprehensive health care. Don’t wait—act now to cover yourself and your family.”

Why Choose Peak and Elevate?

Both organizations operate as nonprofits, reinvesting in their communities rather than focusing on profits for private shareholders. They prioritize a partnership that delivers high-quality, affordable coverage options to Coloradans. This allows them to offer:

  • $0 preventive care
  • $0 to low-cost primary care and mental health counseling visits
  • A robust network of local providers, including major hospitals such as Pagosa Springs Medical Center, St. Anthony Summit, and Mercy Hospital
  • Expanded access to over 3,000 mental health counselors through a partnership with SonderMind

The Nonprofit Difference

Since 2019, Peak Health Alliance has worked to make health insurance more accessible to Coloradans. By analyzing claims data and fostering collaboration between local communities, hospitals, and insurance providers, the nonprofit has saved members in eight rural counties more than $16.1 million.

Act Before Jan. 15

Don’t miss this critical opportunity to secure coverage for 2025. Enroll by Jan.15 to avoid missing out on health insurance for the year. Peak Health Alliance and Elevate Health Plans are here to help you every step of the way.
